Best sugar substitutes for diabetics in India: the 2026 guide

For diabetics in India, the sweeteners with no measurable effect on blood glucose are monk fruit, allulose and stevia. Allulose goes further: a 2024 meta-analysis in Metabolism Open found it lowered post-meal glucose in people with type 2 diabetes. Jaggery, honey and coconut sugar raise blood glucose the way sugar does.

India has 101 million adults living with diabetes and another 136 million with prediabetes, according to the ICMR-INDIAB national survey published in The Lancet Diabetes & Endocrinology in 2023. Most advice aimed at those 237 million people is about what to stop eating. This guide covers the sugar substitutes for diabetics in India that are worth buying, and the ones that are sold on a number nobody measured.

What actually makes a sweetener safe for a diabetic?

A sweetener is safe for a diabetic when it adds no digestible carbohydrate to the food you eat. Glycemic index is the number every pack quotes, but ISO 26642, the international standard that defines GI testing, says foods with under 10 g of glycaemic carbohydrate per serving should not be tested at all.

Which means the "GI: 0" on a pouch of stevia, monk fruit, erythritol or allulose was never measured in a lab. It's a zero by arithmetic. No carbohydrate in, no glycaemic response out.

That includes ours. Saying so costs us nothing, because the entire category quotes that zero as if it were a competitive result, and it's a property every zero-calorie sweetener shares.

So GI tells you very little once you're comparing one zero-calorie sweetener against another. Three things tell you more.

  • What else is in the pouch. A "stevia" sachet is mostly bulking agent by weight. If that bulking agent is maltodextrin, you're eating something with a glycemic index higher than table sugar, sold to you as a diabetic product.
  • How much you can eat before your gut objects. Every sugar alcohol and rare sugar has a ceiling. Above it, you get bloating and worse.
  • What it does inside the recipe. A sweetener that won't dissolve in chai or won't brown in halwa gets abandoned by week three, and the sugar comes back.

Which sugar substitutes for diabetics in India are actually safe?

Monk fruit, allulose and stevia have no measurable effect on blood glucose or insulin in humans. All three pass through without being metabolised for energy. Erythritol and sucralose also register zero on glucose, but both carry evidence worth reading before you commit to them daily, covered further down.

Monk fruit

Monk fruit gets its sweetness from mogrosides, which are 150 to 250 times sweeter than sugar by weight. The body doesn't break them down for energy, so there's no glucose response and no insulin response. FDA has issued "no questions" letters in response to multiple GRAS notices for monk fruit extract, and no regulator has set an acceptable daily intake for it.

The catch is that monk fruit is almost never sold alone. At 200 times the sweetness of sugar, a spoon of pure extract would be unusable, so it's blended with something bulky. What it's blended with is the actual decision you're making.

Stevia

Stevia is the most widely available zero-GI sweetener in India and it works. Steviol glycosides aren't metabolised for energy and don't move blood glucose.

Two things to know. EFSA has set an acceptable daily intake of 4 mg per kg of body weight for steviol glycosides, reaffirmed in November 2024 after declining a request to raise it. And stevia's bitterness lands harder in tannic drinks, which is why it tastes worse in chai than in cold coffee. We've written up how monk fruit and stevia actually compare in more detail.

Allulose

Allulose is a rare sugar that occurs naturally in figs, raisins and jackfruit. It's structurally close to fructose, and the body absorbs it and then excretes it largely unchanged, which is where the roughly 0.2 to 0.4 kcal per gram comes from. It behaves like sugar in a pan, which the other two don't.

Does allulose do more than just not raise blood sugar?

Yes. Allulose is the only sweetener on an Indian shelf with human trial evidence that it lowers post-meal glucose rather than merely failing to raise it. A 2023 meta-analysis in PLOS ONE found 5 g and 10 g doses cut the post-meal glucose curve by roughly 13 to 14%. A 2024 meta-analysis in Metabolism Open found the same effect in people with type 2 diabetes.

The proposed mechanism is inhibition of alpha-glucosidase, the enzyme that breaks starch down into absorbable glucose. Slow that enzyme and the glucose from the rest of the meal arrives more gradually.

Two things are worth being precise about, because precision is what makes the rest of it trustworthy.

The PLOS ONE review was run in healthy adults and deliberately excluded diabetics. Its authors flag that the studies are few and small.

The 2024 type 2 diabetes analysis (Ayesh et al., Metabolism Open) pooled 126 participants across 6 trials. The glucose curve improved and time-above-range fell. Fasting glucose and time-in-range didn't move.

So the effect on what happens after a meal is real and repeated across two independent analyses. It's a food ingredient doing something useful, not a medicine, and we're not going to pretend otherwise.

Is erythritol still safe for diabetics?

Erythritol has a glycemic index of 0 and 30 years of safety data behind it. In 2023 a study in Nature Medicine linked higher blood erythritol to cardiovascular events, and FDA's own evaluation of that paper disputes the reading. The evidence is unsettled, which matters more for a diabetic than for anyone else.

The study, by Witkowski and colleagues at the Cleveland Clinic, found that people with higher circulating erythritol had more cardiovascular events, and showed erythritol made platelets more likely to clot.

FDA published a formal evaluation and pushed back on three points. The cohorts were already high cardiac risk rather than healthy. Dietary erythritol intake was never measured in any of them. And because the body makes erythritol from glucose through the pentose phosphate pathway, blood levels may be reporting sugar intake rather than sweetener intake.

So it isn't settled either way. What matters here: people with diabetes already carry elevated cardiovascular risk, and erythritol is the bulking agent in most monk fruit sweeteners sold in India. If you're buying a monk fruit pouch, turn it over. A lot of what's marketed as monk fruit is 99% erythritol by weight.

What about xylitol, sucralose and aspartame?

Xylitol has a glycemic index of 7 and 2.4 kcal per gram, so it isn't a zero option, and a 2024 study in the European Heart Journal found it prothrombotic. Sucralose and aspartame don't raise glucose directly, but two randomised trials have found sucralose reduces insulin sensitivity. None of the three is a first choice for a diabetic.

On xylitol: the same Cleveland Clinic group that ran the erythritol work published a 2024 paper in the European Heart Journal finding higher plasma xylitol associated with cardiovascular events, and showing enhanced platelet reactivity. It carries the same causality caveat as the erythritol work. But xylitol also has real calories and a non-zero GI, so a diabetic is taking on an unsettled risk for a sweetener that isn't doing the job cleanly anyway. It's also acutely toxic to dogs, which matters in a house with one.

On sucralose: Pepino and colleagues, in Diabetes Care in 2013, found that drinking sucralose before a glucose load produced higher peak glucose and higher insulin in people with obesity. Romo-Romo and colleagues, in the American Journal of Clinical Nutrition in 2018, found 14 days of sucralose reduced insulin sensitivity in healthy adults. Neither is enormous. Both point the wrong way for someone whose insulin sensitivity is the problem.

One thing worth correcting, because it gets quoted at diabetics constantly. WHO's 2023 guideline advising against non-sugar sweeteners for weight control is a conditional recommendation, it covers stevia as well as the artificial ones, and it explicitly excludes people with pre-existing diabetes. Anyone using it to tell a diabetic to avoid sweeteners is citing a document that carved that person out.

Are jaggery, honey, dates and coconut sugar safe for diabetics?

Jaggery, honey and coconut sugar all raise blood glucose substantially and none is a safe substitute for a diabetic. Jaggery's glycemic index is around 84, higher than table sugar at 65. Dates are the interesting exception: their GI is low, but they're roughly two-thirds sugar by weight, so portion is the whole argument.

Jaggery (gud). GI around 84. It carries trace iron and minerals that white sugar doesn't, and that's where the health halo comes from. Those traces don't change what it does to a glucose meter, and it does slightly more than sugar does.

Honey. GI varies enormously by floral source, from about 35 to 87, and it's commonly quoted at 58. Even at the low end it's a concentrated sugar solution. The antibacterial properties are real and irrelevant to glycaemic control.

Coconut sugar. GI around 35 to 54 depending on the study, so genuinely lower than sugar, and about 4 kcal per gram of mostly sucrose. Lower isn't zero, and the gap gets erased the moment anyone uses it spoon for spoon.

Dates. Alkaabi and colleagues, in Nutrition Journal in 2011, tested five varieties and found low glycemic indices across all of them, including in diabetic subjects. Dates are a better sweetener than jaggery. They're also about 67 g of sugar per 100 g, so a date paste barfi is still a sugar barfi.

Mishri. Crystallised sucrose. Chemically the same thing as table sugar.

"Natural" describes where a sugar came from. It says nothing about what your pancreas has to do about it.

How do the sweeteners compare, side by side?

Here is every sweetener on an Indian shelf against glycemic index, calories and the thing you actually need to watch for. The zeros in the GI column are label values, not lab results, for the ISO reason in the first section.

Which sweetener should you actually buy in India?

Buy a monk fruit and allulose blend with nothing else in it. Monk fruit gives clean sweetness at zero glucose response. Allulose gives the bulk, dissolves in hot liquid, browns and caramelises, and is the only sweetener with trial evidence that it lowers post-meal glucose. Nothing else on an Indian shelf does both jobs.

That combination matters because of where Indian sugar actually goes. Chai, filter coffee, halwa, kheer and mithai all need a sweetener that behaves like sugar physically, not one that only tastes sweet.

Stevia goes bitter against tannin. Erythritol leaves a cooling finish and won't caramelise. Allulose does the physical work.

What should you check on the pack before you buy?

Check four things on the ingredients list, not the front of the pack: whether maltodextrin or dextrose is in there, whether erythritol is doing the bulking, whether anything in it carries a daily intake ceiling, and whether it behaves like sugar in a pan. Most pouches fail at least one.

Turn the pack over. Here's what each of those four is telling you.

  1. Is maltodextrin or dextrose in there? Both raise blood glucose, maltodextrin faster than sugar does. They're in a large share of cheap stevia and sucralose sachets as bulking agent, which means most of what you're eating by weight is the thing you're trying to avoid.
  2. Is erythritol the bulk? Most monk fruit pouches in India are 99% erythritol by weight, which makes them erythritol products with a monk fruit label. Given the unresolved cardiovascular question above, that's a choice worth making deliberately rather than by accident.
  3. Is there a daily intake ceiling on anything in it? Stevia carries EFSA's 4 mg/kg limit. Monk fruit and allulose carry none.
  4. Will it behave like sugar in the pan? If it won't brown or dissolve, it fails at halwa, kheer and mithai, and it gets abandoned.

Three things to know before you buy, ours included.

It costs more, and there's no way around that. Monk fruit and allulose are expensive raw materials. A pouch bulked out with maltodextrin or erythritol will always be cheaper per gram, because those cost almost nothing. That's the most common objection we get, and the honest answer is that you're paying for what isn't in the pack.

It sits easier than the sugar alcohols. Allulose is a rare sugar, not a polyol. It's absorbed in the small intestine and excreted, rather than fermented in your colon the way sorbitol, maltitol and lactitol are, and all three of those have lower tolerance thresholds than allulose does.

Han and colleagues, in Nutrients in 2018, put allulose's maximum daily intake at 0.9 g per kg of body weight. That's roughly 54 g a day for a 60 kg adult. Nobody sweetens their food with 54 g of anything.

Individual tolerance varies, as it does with every food. Start where you'd normally use sugar and you'll be nowhere near that figure.

It won't taste identical to sugar, and no zero-calorie sweetener does. Some people find a monk fruit and allulose blend slightly less sweet than they expected. What it doesn't have is the cooling effect and chemical finish that erythritol and some artificial sweeteners leave behind.
